What does Additional Life Insurance mean at BVSD?
Benefit eligible employees are able to purchase Additional Life insurance coverage up to $150,000 above the $20,000 life policy provided by the district. The initial enrollment period is now through October 31. Coverage starts January 1, 2011.
Do I need to have a physical?
No, if you apply when you are initially eligible (by October 31).
What are the exclusions?
The only exclusion for this policy is for death resulting from suicide or other intentionally self-inflicted injury.
What is the cost?
The monthly cost is shown on page 7 of the booklet. The cost increases with age. The booklet is available on this web page.
What happens to the coverage when I leave BVSD?
If your insurance ends because your employment terminates, you may be eligible to buy group life insurance (or convert to an individual policy) from The Standard through the Portability or Conversion provisions, assuming you meet the eligibility requirements. The application and eligibility requirements are available on this web page.
Can I purchase coverage for my spouse, domestic partner, and/or children?
Yes, during this initial enrollment period, you may elect Additional Life coverage for a spouse or domestic partner in amounts of $10,000, $20,000 or $30,000, not to exceed 100 percent of your Additional Life coverage. You may elect $10,000 of coverage for your eligible children.
Is Accidental Death and Dismemberment (AD&D) coverage also included?
Yes, this plan includes AD&D insurance. With AD&D insurance, you or your beneficiaries may be eligible to receive an additional amount in the event of death or dismemberment as a result of an accident. The amount of this AD&D insurance benefit for loss of life is equal to the Additional Life insurance coverage on the date of the accident.
The AD&D insurance benefits have the following exclusions (these exclusions apply only to the AD&D benefit and not to the Additional Life Policy):
• War or act of war, declared or undeclared, whether civil or international, and any substantial armed conflict between organized forces of a military nature
• Suicide or other intentionally self-inflicted injury
• Committing or attempting to commit an assault or felony, or actively participating in a violent disorder or riot
• Voluntary use or consumption of any poison, chemical compound, alcohol or drug, unless used or consumed according to the directions of a physician
• Sickness or pregnancy existing at the time of the accident
• Heart attack or stroke
• Medical or surgical treatment for any of the above
How do I enroll?
Complete the enrollment/beneficiary form at the back of the booklet (or download from this web page) and return to Human Resources before October 31. Even if you choose not to purchase Additional Life insurance, you can use the same form to update your beneficiary information for the district $20,000 policy. It is part of the same policy.
