Registration packets are available from the Foothill Office. Completed registration packets should be returned to the Foothill Office. Completed Open Enrollment packets must be returned to the Open Enrollment Office.
Registration packets must include:
- BVSD Enrollment Forms
- A copy of your child's legal birth certificate (not hospital souvenir copy)
- Immunization records
- Proof of residence - 2 forms required from the list below
- Contract for purchase
- Lease
- Utility bill
- Utility Service Verificaton Letter
- Recent physical examination results form
- We can only accept complete registration packets!
A physical is required prior to school starting.
