INTERSCHOLASTIC ATHLETIC AND EXTRACURRICULAR ACTIVITIES
POLICY ACKNOWLEDGMENT

I,                                           , desire to be a participant in the  Boulder Valley School District’s interscholastic athletic and extracurricular activities programs. My signature acknowledges the following:

1. I have read and understand this Policy, including the Student Code of Conduct, and agree to comply with it.
2. I understand these expectations are in place throughout my participation in interscholastic athletics and extracurricular activities.
3. I reside in the attendance area for the school listed above, or I have received permission from appropriate school authorities to attend a school or participate in athletics or activities in a
   school not in the attendance area of my residence.
4. I hereby authorize transportation to and from scheduled events on school vehicles or private vehicles in accordance with the School District policy and regulation for Field Trips
   (IICA and IICA-R).
