DIRECTORY INFORMATION OPT OUT
This form is to be used by parents/guardians or eligible students that wish to opt out of sharing Directory Information as detailed below. Please refer to BVSD’s Student Information Privacy website for more information.
Directory Information is defined by Board Policy JO as:
- Student name and birth date;
- Participation in officially recognized sports and activities;
- Height and weight of athletic team members;
- Dates of attendance and enrollment status;
- Schools attended and grade level;
- Degrees, awards, honors, or other distinctions received; and,
- Photographic, video, or electronic images maintained by the School District of students in the classroom or at other school and district functions.
The School District may also release the address, School District email address, and telephone number of high school students to military recruiters, postsecondary institutions, and prospective employers.

Directory Information, that may be used for school-sponsored BVSD publications, media, and School District‐connected groups, organizations, or individuals for authorized purposes consistent with Board Policy and applicable law. These include, but are not limited to: social media, school yearbooks, school newsletters, local newspapers, and PTO/PTA. |

I understand that this request takes effect when it is submitted and does not rescind any prior sharing of directory information.
