DIRECTORY INFORMATION OPT OUT (9-12)

This form is to be used by parents/guardians or eligible students in grades 9 - 12 that wish to Opt Out of sharing Directory Information. Please refer to BVSD’s Student Privacy website for more information. 

 

I understand that unless I have made the Opt-out selection above and submitted this form to my student’s school:

The school, the School District, and/or outside media may use information, images, or video of my student. 

In the event the school or the School District uses depictions of my student, that no compensation will be made for this use.

My student’s information may be shared with and my student may be contacted by military recruiters, colleges and universities, and/or employers. 
 

I understand that I must submit this form each school year.

I understand that this request takes effect when it is submitted and does not rescind any prior sharing of directory information.
