Process

Parents/guardians who are interested in Transitions Services should
consult their student’s case manager at their home school and/or
teachers at the Transitions Center. Depending upon the goals and needs
of each student, different services may be required. Transitions teachers
are available to help tailor a plan for how to best prepare students for
their transition from secondary school to adulthood. Program activities
are designed to address employment skills, independent living skills, and
post-secondary opportunities.

Schedule

Transitions Services follow the BVSD school calendar. Days and times of attendance vary because students’ schedules are based on their IEP identified transition needs. Students receive a maximum of 21 hours of services per week. The Transitions Center operates programs from 8:25am to 2:45pm Monday Wednesday Friday and 9:30am to 2:45pm Tuesday Thursday, though students only attend 2-4days/week based on needs. Transportation is provided by the school district to and from school and to most job sites. When appropriate, students may access public transportation via RTD, Via, or Call-n-Ride for work experiences. (Most students’ days are divided into morning and afternoon blocks, which may include classes and/or work experiences.)

Programs

BVSD Transition Services offers training to students and information to parents to prepare them for post-school opportunities.
• Students develop work skills in real employment environments with adult supervision.
• Program staff help students acquire daily living skills, such as appropriate social behavior, using public transportation, making and keeping friends, and money management (purchasing items, budgeting, and banking).
• Instruction emphasizes functional academic skills, such as reading and math, which are needed in work and community environments.
• Families receive support in helping to understand the adult services available upon graduation.


Instructional Classes

A variety of instructional classes are available according to individual needs. The Transitions Center implements the high school state standards as they apply to community and work settings, which the student will access as an adult.

Employment Training

Transitions Services works in collaboration with local businesses and nonprofits to provide employment training through unpaid work experiences for students in their communities.

The program provides basic job skills training and on-going support to help students achieve goals that may include the following:
• Working towards independence in a job setting
• Developing social skills in a work environment
• Finding long-term employment
• Maintaining a job or career

Some of the goals of the job-training program are to:
• Gradually increase expectations for accuracy, speed, duration of work, and on-the-job independence to expand students’ employment potential
• Identify work preferences through “job sampling” in a wide variety of employment situations
• Build a resume in collaboration with the student and family that includes these work experiences and any other positions the student has held

Job training for Transitions students is only available during the academic school year. A job coach is provided by BVSD unless it is jointly determined by the business, the student’s case manager, and parent/guardian that this is not necessary. Students and their job coaches do not go to job sites during school holidays, breaks, weekends, and vacations. Most students work 1-2 hour shifts at least three times a week during school hours. Hours are determined according to both the needs of the business and the student’s schedule.

Enrollment Information

Once parents decide to utilize Transitions Services, a transition meeting will be held. This will include the case manager, all relevant service providers, parents/guardians, and the student, if possible. Paperwork required for registration at the Transitions Center includes: Emergency Contact Information, BVSD Transition Services Student Information, Work Training Agreement(s), Permission to Photograph and Videotape, Permission for Community Access, and Social Connections forms. Some job sites such as hospitals may require additional paperwork or medical testing (TB screening, etc.) to be completed before a student may work in their facility.
