Current and Former Student Records
Looking for access to student records including academic transcripts or health documents? Need assistance with education verification? Boulder Valley School District is happy to help.
If the student is currently attending or if the student's last year of attendance in our schools was 7 school years ago or less, the school's registrar will assist with your records/transcript request.
Our Student Enrollment Center can assist with all other records/transcript requests or in the event that the school is closed. Fill out your request form below:

Complete the form electronically, then either scan and email it to firstname.lastname@example.org, or send it through US mail to the address on the top of the form.
Print the form, complete it and then either scan and email it to email@example.com, or send it through US mail to the address on the top of the form.
Our Special Education department handles all student records pertaining to SPED students.
Per District policy and the Family Educational Rights and Privacy Act (FERPA) student records are only available to those who have authorization from the student or parent, guardian or responsible person if the student is under 18 years of age. If the student is 18 years of age or older, school records will not be released to a parent, guardian or responsible person without the student's written consent.
